Pengaruh Penambahan Contract Relax Stretch Pada Intervensi Infrared Terhadap Penurunan spastisitas Pada Pasien post Stroke non hemoragik kronik.
Pendahuluan: Spastisitas merupakan gangguan motorik berupa peningkatan tonus otot yang bergantung pada kecepatan regangan akibat kerusakan Upper Motor Neuron pada pasien pascastroke nonhemoragik kronik. Kondisi ini dapat membatasi lingkup gerak sendi dan aktivitas fungsional. Infrared sebagai pemanasan superfisial dapat meningkatkan aliran darah dan fleksibilitas jaringan sebelum peregangan. Contract Relax Stretch (CRS) dapat memfasilitasi relaksasi otot melalui autogenic inhibition. Penelitian ini bertujuan menganalisis pengaruh penambahan CRS pada intervensi IR terhadap penurunan spastisitas pasien pascastroke nonhemoragik kronik.
Metode: Penelitian ini menggunakan desain pre-experimental dengan pendekatan one group pretest-posttest. Sampel terdiri atas 15 pasien pascastroke nonhemoragik kronik di Rumah Sakit Islam PKU Muhammadiyah Pekajangan yang dipilih menggunakan total sampling. Tingkat spastisitas diukur menggunakan Modified Ashworth Scale (MAS) sebelum dan sesudah intervensi. Intervensi IR diberikan sebagai pemanasan awal, kemudian dilanjutkan CRS, dengan frekuensi dua kali seminggu selama empat minggu, sehingga total terdapat delapan sesi. MAS digunakan untuk menilai perubahan tonus otot dan memiliki validitas serta reliabilitas yang telah dilaporkan memadai dalam penilaian spastisitas. Data dianalisis menggunakan Wilcoxon Signed-Rank Test dengan tingkat kemaknaan p<0,05.
Hasil: Nilai rata-rata (mean) tingkat spastisitas sebelum intervensi adalah 2,67 dengan standar deviasi 0,617 dan termasuk kategori sedang. Setelah delapan sesi intervensi, rerata spastisitas menurun menjadi 1,20 dengan standar deviasi 0,561 dan termasuk kategori ringan. Analisis menggunakan Wilcoxon Signed-Rank Test menunjukkan nilai Z sebesar -3,508 dengan p=0,000 (p<0,05). Temuan tersebut menunjukkan adanya perbedaan tingkat spastisitas yang signifikan antara sebelum dan sesudah pemberian IR yang ditambahkan CRS pada pasien pascastroke nonhemoragik kronik.
Diskusi: Penambahan CRS setelah IR menunjukkan penurunan spastisitas yang signifikan dan dapat menjadi pendekatan fisioterapi yang relevan untuk meningkatkan pengelolaan spastisitas pascastroke kronik.
