PENGARUH PENAMBAHAN STATIC CONTRACRTION PADA INTERVENSI ULTRASOUND DAN INFRARED TERHADAP KEKUATAN OTOT OSTEOARTHRITIS KNEE BILATERAL

Pengaruh Penambahan Static Contraction Pada Intervensi Ultrasound Dan Infrared Terhadap Kekuatan Otot Osteoarthritis Knee Bilateral

Pendahuluan: Osteoarthritis (OA) knee bilateral merupakan penyakit degeneratif sendi penyebab utama disabilitas fisik secara global, dengan lebih dari 600 juta penderita di dunia. Manifestasi klinis yang paling signifikan adalah penurunan kekuatan otot quadriceps akibat Arthrogenic Muscle Inhibition (AMI), yang menciptakan siklus patologis nyeri–disuse–atrofi. Intervensi konvensional Ultrasound (US) dan Infrared (IR) efektif menurunkan nyeri namun belum memberikan stimulasi mekanik yang optimal untuk peningkatan kekuatan otot, sehingga diperlukan penambahan Static Contraction sebagai stimulus neuromuskular aktif. Penelitian ini bertujuan mengetahui pengaruh penambahan Static Contraction pada intervensi Ultrasound dan Infrared terhadap kekuatan otot pada penderita Osteoarthritis Knee Bilateral di RSI PKU Muhammadiyah Kabupaten Tegal.
Metode: Penelitian ini menggunakan desain quasi-experimental pre-test post-test tanpa kelompok kontrol dengan pendekatan accidental sampling. Sebanyak 20 responden dilibatkan, menjalani intervensi selama 4 minggu dengan frekuensi 3 kali per minggu (12 sesi), terdiri atas Infrared (15 menit), Ultrasound (frekuensi 1 MHz, intensitas 1,5 W/cm² selama 8 menit), serta Static Contraction berupa Quadriceps Setting Exercise dan Short Arc Quads (10 repetisi, tahan 6–10 detik, 2–3 set). Kekuatan otot diukur menggunakan Manual Muscle Testing (MMT) Skala Lovett. Analisis data menggunakan uji Shapiro-Wilk dan uji Wilcoxon Signed-Rank Test.
Hasil: Dari 20 responden, 14 orang (70,0%) perempuan dan mayoritas kelompok usia pra lansia (65,0%). Nilai rerata MMT meningkat signifikan dari 3,50 (SD=0,513) menjadi 4,55 (SD=0,510), dengan uji Wilcoxon menghasilkan Z = -4,379 dan p = 0,000 (p<0,05).
Diskusi: Terdapat pengaruh signifikan penambahan Static Contraction pada intervensi Ultrasound dan Infrared terhadap peningkatan kekuatan otot quadriceps penderita Osteoarthritis Knee Bilateral. Sinergi efek pre-heating IR, efek termal-nontermal US, serta stimulasi neuromuskular aktif dari Static Contraction terbukti memutus rantai AMI tanpa memberikan stres kompresif pada sendi lutut yang mengalami degenerasi.
